Cutthroat II follows the same format as the original Cutthroat with the following differences:
- The teams are pre-determined.
- Rookies: Players who have done 1 or no challenges.
- Veterans: Players who are on their 3rd-6th challenge.
- Ultimates: Players who completed 7+ challenge.
- Voting for the Gulag will be public

14 challenge champions and 14 challengers arrive in Stockholm, Sweden thinking that they'll be competing against each other as two separate teams. Instead, TJ Lavin announces that they will be competing as pairs: one challenger and one champion of the opposite sex. To decide the pairs, a daily mission is held to determine a draft order. Every subsequent challenge, the pairs will be competing for immunity from elimination. The last place team is immediately forced to compete in the elimination round, called "The Crypt."
